Comprehensive Veteran Assistance in North & Northeast Philadelphia

In terms of health and well-being, LP³ veterans reported improvements in physical health due to better nutrition and access to necessary medical and health services. Support services, including peer counseling and recreational activities, contributed to enhanced mental health and overall quality of life. LP³’s efforts resulted in a significant number of veterans transitioning to permanent housing, contributing to long-term stability and independence. Additionally, access to job training and career counseling helped veterans find and maintain employment, further enhancing their self-sufficiency.

We conducted regular surveys and interviews with veterans to gather feedback on their experiences and outcomes related to physical and mental health, housing stability, and overall quality of life. Health improvements were monitored through reports from our case managers. LP³ collected data on the number of veterans served, their access to food, clothing, and shelter, and the number successfully transitioned to permanent housing. Progress toward specific goals, such as meeting immediate basic needs, improving health, and supporting transitions to permanent housing, was tracked using predefined outcomes. Internal evaluations were conducted to assess program effectiveness and identify areas for improvement.

The grant significantly enhanced both the quality of services provided and our organizational effectiveness. With the grant, we expanded our capacity to deliver essential services such as food, transportation, clothing, and shelter, enabling us to support a greater number of homeless veterans. Funding also allowed us to improve the quality and frequency of support services, including nutritious meals, medical care, and mental health support, directly benefiting the veterans we serve.

In terms of organizational effectiveness, the project fostered stronger partnerships with other organizations and agencies, leading to better coordination of services and resources. We are preparing to reopen our previous Torresdale location to operate another affordable housing single-room occupancy program for veterans. Additionally, we are working with the Department of Corrections to secure funding for formerly incarcerated veterans and individuals, with this project set to begin in the spring of 2025.
